ADMIT REQUEST FORM
- Students have 72 hours upon their return to school to receive an admit.
- PERMISSIBLE absences, tardies, and/or early sign-out include:
- Student illness: Students missing 3 or more consecutive days of school due to illness or injury are required to provide a written statement from a health care provider.
- Medical appointment
- Death in the family
- Observance of a religious holiday or service when it is mandated for all members of a faith that such a holiday or service must be observed.
- School-sponsored event.
- Subpoena by law enforcement agency or mandatory court appearance.
- Students accumulating 10 or more excused or unexcused absences MUST submit medical documentation to receive a permissible admit.
