Opening of School Paperwork Due August 21st
- Media Release Form – Indicates your permission regarding the use of your student’s image and likeness in school publications, social media, and promotional materials.
- Student Code of Conduct Acknowledgment – Please review the Student Code of Conduct with your child using the link provided and submit the acknowledgment form indicating that you have read and discussed its contents.
- Optional Opt-Out Form
Emergency Contact Card Due August 21st
Please ensure that the registering parent completes the Emergency Contact Card. Once submitted, updates will be available the next business day. Kindly note that any submission without a valid government-issued ID will be deleted and will need to be resubmitted.
Admit Request (Open only during School Hours)
Admit requests must be submitted for absences/tardies/early releases within 3 days upon returning to school with proper documentation. More than 10 absences (excused or unexcused) must include medical documentation.
Please complete the Free or Reduced-Price Lunch application. In addition to helping determine eligibility for free or reduced-price meals, the application may provide access to other valuable benefits. If you do not complete an application, school lunch will be charged at the full price beginning in September. Please take a few minutes to complete the application today!
Gear Up Consent Forms (10th and 11th Grade ONLY!)
Through Miami Dade College’s GEAR UP, students receive support with college and career planning, academic success, financial aid, and more. It’s a great way to explore opportunities, build confidence, and prepare for life after high school.
Forms must be completed in order to participate in any GEAR UP related event.
